Athlete Eligibility Checklist

1. Register with the NAIA
2. Complete profile and pay registration fee
3. After graduation, request two OFFICIAL transcripts from your High School:
   1. NAIA Eligibility Center - Transcripts

   2. Providence Christian College
      464 E. Walnut St., Pasadena, CA 91101
4. After graduation, request two OFFICIAL SAT/ACT test scores directly from testing service:
   1. NAIA - Code 9876
   2. Providence - SAT Code 7893, ACT Code 6958
5. Physical/Health packet, Medical and Liability packet, Insurance Information, and Concussion Information
   1. Download Physical/Health form and bring to Physical with your doctor; physical must be preformed by: Medical Doctor (MD), Physicians Assistant (PA), Doctor of Osteopathic (DO). 
   2. Download Medical and Liability form and fill out completely
   3. Scan insurance card (front & back)
   4. Email all physical/liability and insurance forms no later than July 31st to: robert.simmons@providencecc.edu
